How do I access the service?

Anyone can make a referral to us, providing the patient has given his or her consent.

You can do this by phone, email or in writing.  For more information click here

What will happen next?

When we receive the referral, we will contact you within 2 working days to make an appointment to visit and explain our details

Ideally, we will visit you in your own home.

This enables us  to

  • get a picture of your home circumstances,
  • meet other family members
  • write directions to your home, which will be useful if you ever need care at short notice

We will tell you about services that might be of interest to you and take quotecorner.com/celexa.html some details.  If you decide you would like us to contact you regularly, we will agree how often this will happen.

What happens when I need to contact  Paul Sartori?

You will have a 24 hour mobile phone contact number.  The person answering this call will be a member of the care management team.  This may be the person who visited you at home.  Whichever member of the team it is, she will have your details with her.

If you have a simple query, she will answer this if she can.  If it is not an appropriate query for us to deal with, she will be able to advise you and give you contact details of the person who should be able to answer your query.

If you need care at short notice, we will probably need to visit again to take more details and update our information.  We will do this the same day if it is urgent.
